The services available through Fraser Health Home and Community Care Services

Presented by Shannon Parsons, RN, Fraser Health currently working as a Regional Clinical Nurse within Home and Community Services. She has over 30 years of experience in community-based nursing, with a strong focus on supporting older adults to remain safe, independent, and well-supported in their homes for as long as possible. This presentation will provide an overview of the services available through Home and Community Care Services, including eligibility, scope of services, and supports available for clients and families. Participants will also learn when and how to access these services, as well as the referral process.
